In the past, many face-lifts were accomplished by tightly pulling the skin up and back. While this did improve sagging and reduce wrinkles in the lower two-thirds of the face, it often gave an unnatural appearance. Now, new techniques can take off years while providing a more natural result. The primary method involves making incisions behind the hairline and the outer ear. Next, the skin is lifted and the facial muscles and underlying fat are re-suspended in a higher position. Excess skin is then removed, usually with an incision in front of the ears. Unlike surgeries that remove fat in the jowl or cheek areas, this technique merely repositions the tissues. The theory behind this method is that sagging jowls and cheeks are actually tissue from the mid-to-upper cheek which has been pulled down by gravity. Therefore, moving tissue up restores a more youthful appearance without overly stretching the skin. Typically, the neck is also lifted at the same time as the face. After making a small incision in the chin, neck muscles are tightened and the neck tissue is adjusted as necessary.
